A ULTRASSONOGRAFIA

A ultrassonografia constitui uma modalidade de exame de imagem sem a utilização da radiação, constituindo um exame inócuo para os pacientes e sem contraindicações ou efeitos colaterais.

É uma técnica de diagnóstico por imagem, utilizada para visualizar estruturas internas do corpo, tais como fígado, vesícula biliar, vias biliares, pâncreas, baço, vasos abdominais, bexiga, próstata, retroperitôneo, estruturas superficiais como a tireóide, glândulas salivares, mamas tendões, músculos, articulações, vasos sanguíneos superficiais e muitas outras novas aplicações que as inovadoras tecnologias podem propiciar. Um grande exemplo são as recentes aquisições tridimensionais 3 D e 4 D e softwares que permitem visualizar microcalcificações nas mamas e nódulos tireiodeanos. Também lançados recentemente a imagem harmônica e o PRECISION IMAGE, que proporciona melhora significativa na acuidade diagnostica.

Para a realização do exame ultrassonográfico é necessário que se aplique uma pequena quantidade de gel sobre a superfície da pele para remover o ar e facilitar o deslizamento e, por meio de um transdutor, a geração, transmissão e recepção de ondas sonoras focadas diretamente nas áreas a serem examinadas.

A obtenção da imagem é realizada pela emissão de ondas sonoras de alta frequência que são refletidas ao atingir um tecido ou órgão estudado. Este fenômeno físico também é conhecido por “eco” e, por isso, é comum encontrarmos o termo ecografia como sinônimo de ultrassonografia.

A ultrassonografia pode ser empregada para o diagnóstico de diversas doenças em diferentes partes do corpo humano. A Ultrassonografia de Abdome Superior é um procedimento não invasivo, utilizado para avaliação, seguimento, diagnóstico e caracterização das alterações do fígado, pâncreas, vesícula biliar, vias biliares, baço, rins, aorta e veia cava inferior, auxiliando, complementando e interagindo com outras especialidades médicas.

É um método que não utiliza nenhum tipo de radiação e não apresenta efeitos colaterais.

Para a realização desse exame é necessário o jejum porque evita que a vesícula biliar se esvazie e permite avaliar de forma adequada órgãos mais profundos, como o pâncreas e a aorta.

Preparo e Orientações para Ultrassonografia do Abdome Superior

Para a realização deste exame será necessário jejum, conforme as seguintes orientações:
- até dois anos de idade: jejum de 3 horas;
- de dois a cinco anos de idade: jejum de 4 horas;
- de cinco a 12 anos de idade: jejum de 5 horas;
- acima de 12 anos: jejum de 6 horas.
Não há necessidade da suspensão de medicações, contanto que sejam tomadas com água. Mesmo que você já tenha feito cirurgia da vesícula o jejum continua sendo obrigatório;
Procure usar roupas confortáveis, preferencialmente de duas peças;

A Ultrassonografia de Abdome Total é um procedimento não invasivo, utilizado para avaliação, seguimento, diagnóstico e caracterização das alterações do fígado, da vesícula biliar, dos rins, do pâncreas, da bexiga, dos grandes vasos, do retroperitônio e, eventualmente, do trato gastrointestinal, auxiliando, complementando e interagindo com outras especialidades médicas.

É um método que não utiliza nenhum tipo de radiação e não apresenta efeitos colaterais. Além do jejum – necessário porque evita que a vesícula biliar se esvazie e permite avaliar de forma adequada órgãos mais profundos – a repleção da bexiga também é essencial, pois desloca as alças intestinais e atua como uma janela para a transmissão das ondas ultrassônicas, facilitando a visualização e avaliação dos órgãos e estruturas da região abdominal.

Preparo e Orientações para Ultrassonografia de Abdome Total(Este exame necessita de preparo prévio)

Para a realização deste exame será necessário jejum, conforme as seguintes orientações:
- até dois anos de idade: jejum de 3 horas;
- de dois a cinco anos de idade: jejum de 4 horas;
- de cinco a 12 anos de idade: jejum de 5 horas;
- acima de 12 anos: jejum de 6 horas.
Não há necessidade da suspensão de medicações, contanto que sejam tomadas com água. Mesmo que você já tenha feito cirurgia da vesícula o jejum continua sendo obrigatório;
Para a repleção da bexiga você deverá tomar água ou chá, observando as seguintes orientações:
- menores de três anos: ingerir uma mamadeira de água ou chá ou dois copos de água 30 minutos antes do exame;
- de três a 10 anos: beber de três a quatro copos de água 1 hora antes do exame;
- acima de 10 anos: beber quatro copos de água 1 hora antes do exame.
Caso tenha alguma dificuldade na ingestão ou na retenção do líquido, deixe para beber a água na clínica. Para isso, chegue com 1 hora de antecedência;

Preparo e Orientações para US de Abdome Superior com Prova de Boyden. Para a realização deste exame será necessário jejum, conforme as seguintes orientações:

Não há necessidade da suspensão de medicações, contanto que sejam tomadas com água. Mesmo que você já tenha feito cirurgia da vesícula o jejum continua sendo obrigatório;
A primeira parte do exame é feita em jejum. Para a segunda parte será necessária a ingestão de um lanche rico em gordura (fornecido pela clínica) para desencadear a contração da vesícula. O exame será finalizado após cerca de 40 minutos, tempo necessário para que a ingestão do lanche estimule o esvaziamento da vesícula. (Prova de Boyden);

Elastografia é uma técnica de diagnóstico por imagem que tenta detectar o grau de “rigidez” do tecido analisado.

Baseia-se no fato de que lesões malignas tendem a ser mais rigidaz do que os tecidos vizinhos e de que fígados saudáveis apresentam menor consistência.

Existem várias modalidades de elastografia, seja por ultrassonografia ou ressonância magnética. As técnicas ultrassonográficas de elastografia mais difundidas são: “Quasistatic Elastography / Strain imaging”; “Acoustic Radiation Force Impulse Imaging (ARFI)”; “Shear Wave Elasticity Imaging (SWEI)”; “Supersonic Shear Imaging (SSI)” e “Transiente Elastography”.

A utilidade da elastografia da mama foi confirmada especialmente em pequenos nódulos, cistos complexos ou cistos com componentes sólidos.

A técnica pode reduzir a necessidade de biópsia em lesões classificadas como BI-RADS 3.

O método possui um papel significativo na condução de nódulos com dimensões inferiores a 5,0 mm que são detectados pela ultrassonografia, mas não pela mamografia. Nestas situações, é comum que a paciente seja submetida a biópsia.

A elastografia avalia as propriedades elásticas do tecido e graças ao curto tempo de exame, a caracterização em tempo real das lesões, a interpretação imediata, o baixo custo e adoção de critérios de interpretação de imagem validados, ela tem se mostrado útil na prática clínica.

Elastografia é uma técnica de diagnóstico por imagem que tenta detectar o grau de “rigidez” do tecido analisado.

Ela é utilizada para avaliar os nódulos tireoideanos, fornecendo informações sobre a elasticidade do tecido, com base no fato de que processos patológicos como o câncer alteram as características físicas do nódulo.

As medidas da elastografia são feita durante o exame de ultrassom, utilizando o melhor aparelho e sonda.

O médico exerce pressão suave com a sonda e seleciona a parte da imagem que inclui o nódulo a ser avaliado.

Devem ser adquiridas duas imagens ultrassonográficas: uma antes e uma depois da compressão do nódulo e acompanhar o deslocamento do tecido pela avaliação do feixe, fornecendo a medida acurada da distorção do nódulo.

O elastograma da tireoide é gravado sobre uma imagem ultrassonográfica típica – aquela em escala de cinzas – utilizando uma escala de cor e classificado usando o score de elasticidade.

Para minimizar a variabilidade inter e intraobservador, a compressão à mão livre aplicada na região do pescoço é padronizada pela medida em tempo real gravada em uma escala numérica para manter o nível intermediário ótimo para a avaliação elastográfica.

Elastografia é uma técnica de diagnóstico por imagem que tenta detectar o grau de “rigidez” do tecido analisado.

Especialmente no caso da análise do fígado, ela permite avaliar o grau de fibrose de pacientes com hepatopatia crõnica.
A técnica afere a velocidade do som passando através do fígado e a converte em medida de rigidez hepática.

O objetivo é evitar a biópsia hepática. Durante muito tempo, a biópsia foi o padrão ouro na avaliação da fibrose do fígado. Sempre foi muito utilizada para avaliar pacientes com hepatites virais (especialmente por vírus B e C), para estadiar a doença e determinar quais pacientes devem receber tratamento. As desvantagens da biópsia residem no fato de se tratar de um procedimento invasivo, requerer hospitalização por meio dia, ser muito cara e estar associada a alguns riscos como dor e sangramento (apesar de bastante infrequente, quando ocorre, pode trazer um risco importante).

Além disso, a biópsia é de um pequeno fragmento do fígado, o que pode conduzir a um estadiamento incorreto se não ela for representativa de todo o fígado. Dessa forma, a biópsia pode trazer um erro na coleta da amostra, o que pode estadiar o grau de fibrose para cima ou para baixo – estes erros de amostra ocorrem em 25-30% das biópsias. Outra limitação da biópsia hepática é que patologistas diferentes podem interpretar de maneira variada a amostra, o que pode resultar em estadiamentos discrepantes.
Dadas estas limitações e o desejo dos pacientes de evitar um teste invasivo, pesquisadores empreenderam muito esforço nos últimos dez anos para desenvolver um teste não invasivo que avalie fibrose.

Elastografia é este teste e possui várias vantagens comparadas à biópsia hepática. Por ser um teste não invasivo, não tem dor e não requer sedação. Além disso, o exame dura cerca de 15 minutos e tem um custo muito menor do que a biópsia hepática e não foi associado a nenhum efeito coleteral. Por fim, os resultados são instantâneos e os clínicos podem usar as medidas para mudanças de conduta rapidamente.

Aqui no CEUS utilizamos a técnica de “Shear Wave” para fornecer uma avaliação fidedigna e quantitativa do grau de fibrose hepática.

A Ultrassonografia de Mamas é um importante método auxiliar no diagnóstico das lesões e/ou alterações mamárias.

Ela é o principal exame complementar à mamografia.

É um método que não utiliza nenhum tipo de radiação e não apresenta efeitos colaterais.

O exame ultrassonográfico informa sobre o estado da pele e dos tecidos subcutâneo, fibroglandular e muscular posterior.

Seus melhores resultados são obtidos em mamas muito densas (mulheres jovens), que por suas características podem “esconder” uma lesão.

Este método é indicado ainda na diferenciação de lesões sólidas e líquidas (cistos), em processos inflamatórios e na monitoração de biópsias e punções aspirativas.

A Ultrassonografia para Monitoração da Ovulação é um exame em que são realizadas ultrassonografias seriadas (em geral três exames), em dias alternados, para que seja definido o dia mais provável da ovulação.

O primeiro exame é feito por via suprapúbica (via abdominal) e por via transvaginal (a etapa por via abdominal é importante para que seja feita uma avaliação da bexiga, útero e ovários, além da pesquisa de lesões adjacentes a esses órgãos; a etapa por via transvaginal complementa as informações obtidas na etapa por via abdominal, oferecendo maior resolução e sensibilidade para a avaliação dos ovários e do útero).

Para este exame a repleção da bexiga é essencial, pois desloca as alças intestinais e atua como uma janela para a transmissão das ondas ultrassônicas, facilitando a visualização e avaliação da área a ser examinada.

Os exames subseqüentes serão realizados apenas por via transvaginal.

O primeiro exame é realizado preferencialmente entre o 10º e o 12º dia do ciclo menstrual, ou a critério do médico solicitante.

Os demais exames serão realizados dois e quatro dias após o primeiro exame.

Preparo e Orientações para Ultrassonografia para Monitorização da Ovulação

(Este exame necessita de preparo prévio apenas para o primeiro exame.)

Para realizar o primeiro exame você deverá tomar quatro copos de água ou outro tipo de líquido não gasoso, 1 hora antes do horário agendado. Caso tenha alguma dificuldade na ingestão ou na retenção do líquido, deixe para beber a água na clínica. Para isso, chegue com 1 hora de antecedência;
Procure usar roupas confortáveis, preferencialmente de duas peças;
Caso tenha exames anteriores relacionados à região a ser examinada pedimos a gentileza de trazê-los. A comparação com exames anteriores proporciona diagnósticos mais precoces e evita, muitas vezes, a realização de incidências adicionais;

Este exame é realizado por ultrassonografia no 1º trimestre de gestação (da fase inicial até a 12ª semana).

A ultrassonografia não emite radiação ionizante, sendo totalmente segura em gestantes.

Por meio da avaliação da translucência nucal e de outros marcadores do primeiro trimestre é possível quantificar e estimar o risco individual para cromossomopatias, como Síndrome de Down, Síndrome de Edwards e Síndrome de Patau.

Utilizando o recurso adicional de imagens tridimensionais (3D), o Morfológico de 1º Trimestre permite avaliar a vitalidade ovular, e verificar a existência de qualquer alteração que possa comprometer o desenvolvimento da gestação.

Preparo e Orientações para Ultrassonografia Morfológica de 1º Trimestre (Este exame não necessita de preparo prévio)

Exame realizado no 1º trimestre de gestação;
É recomendável que você se alimente (lanche ou refeição) de 30 a 60 minutos antes da hora agendada para o exame, pois o feto torna-se mais ativo, facilitando a sua avaliação;

Este exame é realizado por ultrassonografia, com o recurso adicional de imagens tridimensionais (3D e 4D), nos 2º e 3º trimestres de gestação.

A ultrassonografia não emite radiação ionizante, sendo totalmente segura em gestantes.

Segundo trimestre (da 13ª até a 24ª semana): Conhecido como ultrassom morfológico de segundo trimestre, preconiza-se que este exame seja realizado entre a 18ª e 24ª semana de gestação.

Nessa fase pode-se avaliar a morfologia e o aspecto da anatomia fetal, que é medido e documentado, assim como o comprimento do colo uterino.

Terceiro trimestre (da 25ª até o final): Nesse período reavalia-se a anatomia, obtendo-se uma visão mais fidedigna de alguns órgãos, em especial, rins e coração fetal.

A Ultrassonografia Pélvica por Via Abdominal (suprapúbica) é um procedimento não invasivo, utilizado para avaliação, seguimento, diagnóstico e caracterização das alterações e/ou lesões dos órgãos pélvicos – bexiga, útero e ovários -, auxiliando, complementando e interagindo com outras especialidades médicas.

Além disso, é utilizado também para identificar a presença de gestação e avaliar o posicionamento de DIUs.

É um método que não utiliza nenhum tipo de radiação e não apresenta efeitos colaterais.

Preparo e Orientações para Ultrassonografia Pélvica Feminina por Via Abdominal

(Este exame necessita de preparo prévio)

Para realizar este exame você deverá tomar água ou outro tipo de líquido não gasoso, observando as seguintes orientações:
- menores de três anos: ingerir uma mamadeira de água ou chá ou dois copos de água 30 minutos antes do exame;
- de três a 10 anos: beber de três a quatro copos de água 1 hora antes do exame;

A Ultrassonografia Pélvica por Via Transvaginal é um procedimento que se beneficia da menor distância entre o transdutor e a pelve, o que facilita a perfeita visualização, identificação e caracterização das alterações e/ou lesões do útero e ovários, auxiliando, complementando e interagindo com outras especialidades médicas.

É um método que não utiliza nenhum tipo de radiação e não apresenta efeitos colaterais.

Em tese, o exame por via transvaginal dispensa a repleção da bexiga, mas no geral, para que se obtenha um diagnóstico mais preciso, incluindo a pesquisa de lesões na bexiga, este procedimento é precedido de uma investigação por via abdominal (suprapúbica).

Por esta razão, a repleção da bexiga é essencial, pois desloca as alças intestinais e atua como uma janela para a transmissão das ondas ultrassônicas, facilitando a visualização e avaliação dos órgãos e estruturas da região pélvica.

Tão logo essa investigação tenha sido concluída, a bexiga é esvaziada para que se proceda ao exame por via transvaginal.
